Fertility & Pregnancy Considerations

How Weight Affects Fertility

Obesity can have a significant impact on reproductive health. Many women with obesity experience irregular menstrual cycles, hormone imbalances, and ovulation issues, often making it harder to conceive. In fact, obesity has been linked to:

  • 3x higher rates of menstrual disturbances
  • Increased risk of infertility
  • Higher miscarriage rates
  • Greater complications during pregnancy and delivery

This is due to excess fat tissue influencing hormonal function, including producing excess oestrogen-like substances that can inhibit ovulation.

How Weight Loss Can Help

Weight loss can significantly improve fertility outcomes. Even modest weight loss (as little as 7–10kg) has been shown to:

  • Improve ovulation and regulate menstrual cycles
  • Increase chances of natural conception
  • Lower the risk of miscarriage
  • Improve pregnancy outcomes and reduce the risk of gestational diabetes and hypertension

Pregnancy After Bariatric Surgery

Many patients who have struggled with infertility find it easier to become pregnant after weight loss surgery. However, timing is crucial.

We recommend:

  • Delaying pregnancy for 12–18 months after surgery or until your weight has stabilised.
  • Using contraception (preferably non-oral methods) during the rapid weight loss phase.

Becoming pregnant too soon after surgery may lead to complications due to nutritional deficiencies and rapid weight loss.

Your Nutritional Needs During Pregnancy

Women who become pregnant after bariatric surgery will need:

  • 4–6 small meals per day to increase calorie intake
  • Regular blood tests to monitor key nutrients (iron, B12, folate, calcium, vitamin D)
  • Ongoing support from their bariatric care team, including physician and dietitian guidance

Improving Fertility-Related Conditions

Weight loss can also help manage conditions like:

  • Polycystic Ovary Syndrome (PCOS): Many women with PCOS experience improved fertility and more regular cycles after surgery.
  • Insulin resistance and metabolic syndrome: These can contribute to hormonal imbalances and fertility issues, which may improve with weight loss.
